I got a custom one of a kind that’s on this BEAUTIFUL green cordovan leather from Form Function Form. 
 

Bushwood CC, 1980 Caddy Tournament Champion with NOONAN on the top (the part that sticks out of your back pocket). 
 

The ONLY problem I have with almost every yardage book is that they are JUST a tiny bit too snug and end up fraying the paper a lot/are hard to get in. Will loosen up over time I’m sure.

False!  They look great but the one i got did not fit a scorecard and was terrible.  Go with a Buck Magnussen and you will be happy.

I made my own with $20 worth of leather material and supplies from Hobby Lobby....genuine leather, minimal design, works perfectly.  Took me about 3 hours to make.

For those of you in Ontario, Tundra Leather in Hamilton made a nice yardage book holder.  Small place, so don't think they will do bulk, but very reasonable pricing and Sean is a good guy to deal with.

Maybe it's just me but I want to tear my hair out when I see the pencil holder in any location other than in the fold.
