Putting - always going left - slow motion video

Where is your intended start line here? Without seeing your shoulder alignment it looks like you're trying to aim a few feet right of the flag, but your stroke re-routes pretty significantly to the left and the ball starts a foot left of the flag, so several feet left of where your feet are aiming. If your shoulder alignment is off here and is more in line with where the ball is starting then you just have an alignment problem where your hands/arms are just re-aligning with your shoulders. We'd need a full shot to confirm that though.

Looks like your right forearm is rotating over the top of your left forearm as you come through.

 

Tried to replicate what is happening here, and I could do it if I opened up my left shoulder as I came forward.  Maybe that's what's going on?

 

Also, if you're going for an arced putter path and you don't rotate your shoulders at all on the backswing, but you do rotate them on the downswing, you end up with a similar result.

It was a straight, uphill putt. Yep, alignment is a big issue.

I'd guess your shoulders must be aimed at least a little left as your forearms are pointing left of where your feet are aiming. Your "on top" right hand grip isn't helping either since your natural wrist hinge is going to tend to get you going outside-in and shut face. Maybe try strengthening the right-hand grip to get it feeling like it's working under the left hand, or try left-hand-low.

Don't know how much you have experimented with different putters, but I can't putt well with anything other than a slant neck.

Withough seeing the shoulders it looks like you fear the right miss/push and do a re-route after the backswing finishes. Look how at the end if the backswing the transition starts wit the the toe end of the clubhead moving horizontal to the right. The backswing looks fine, but the club should start transition back on the same path and then arc, or rotate, left of target after contact.
