Head/upper body sway issue & weight transfer

I have a significant swing flaw on the downswing which is throwing everything off (confirmed with slow-motion video feedback):

 

The only way my brain knows how to transfer my weight to my lead (left) leg is to get my center of mass over my left foot.  The problem this creates, however, is that from the start of the downswing my upper body sways or slides toward the target in order to get that center mass above the left foot.  I can see on video how my head slides 6-8 inches or more away from its address position toward the target.  This = bad.

 

How can I get my weight transferred to my left/lead leg WITHOUT moving my upper body/center of mass toward the target?  

 

Even when I consciously try to keep the head still during the swing, I can still see on video how I'm moving most of my upper body mass to the left so it's out over the left leg.  If I REALLY do a nice job of keeping my head back/still, then I never get the weight off my right leg and I'm hitting from my back foot.

How is your hip and shoulder mobility? Could be an issue of not getting upper/lower body separation.

Some ways (or via a combination of ways):

 

1. Don't move your upper body back over your trail leg in the backswing -- and make sure you're not getting you weight out to or beyond the outside of your right foot

2. Narrow your stance some if it's too wide

3. Make sure that your right hip moves behind you and around toward the target in the backswing (and that your left hip doesn't move toward the ball and around toward your trail leg)

4. Make sure that your left hip moves back away from the ball and that you get pressure into your left heel early in the downswing (and that you don't just slide your hips toward the target)

 

Posting that video will get you answers specific to what you're actually doing.

As backswing is ending, or as close as I can to that.  At the latest, right as my backswing JUST starts, my upper body moves toward the target to load weight over the left leg.

Mobility and flexibility are good.  I just learned it this way (because when I started I was DUMB and tried to teach myself, rather than get lessons).
