High hands at impact vs. early extension: Chicken or the egg

I've been filming my swings for the last 6 or so months and there's two issues I know are giving me grief. 

1. I early extend towards the top of my backswing. My head raises up and my butt comes forward from my position at address.

2. My hands are higher at impact then they were at address. 

 

I've read that early extension is a symptom and not the actual root cause of the problem. I've had an in person lesson where my instructor told me to focus on getting my hands low, or down as much as I can and that will fix things for me. No matter how hard I try, there's no way I can get my hands back to the impact position. Is this the correct order I should be focusing on things, is it possible to get my hands down at impact without fixing early extension in the backswing first? Video attached for reference...

You've early extended a bunch before you even begin the backswing. So, in the case of the chicken or the egg, I say it's the chicken's grandparents: your setup has your weight too far back toward your heels.
