Hi All. Long time lurker. First time poster from Ireland. 

Current HC is 20. Usual shot is fade more so a slice. Have strengthened the grip which results in more pulls due to the OTT. Have fought the OTT for years. Aware I'm doing it just can't stop myself 🤪

Have had in person and online lessons and seems I always revert back to my old habits.

 

Appreciate any help or tips or drills that could help.

First you must understand how people learn motor skills 

You do so via creating new neural pathways in the brain 

At first these new neural pathways are weak ; to strengthen them required many repetitions .

While one is practicing the new correct pattern , it is common to revert back to your old incorrect patterns , because they are so well ingrained . 
To avoid reverting back to your old habits it is vital to practice  new changes with a swing that is both slower and shorter than  a regular swing . Many famous golfers have practiced their swing changes at super slow motion; other hit balls with eyes closed , others exaggerate the desired motion. 

 

First you need more knee bend at setup

Front of knees should be over the middle of your shoelaces 

 

Second, look at your trail foot at the top compared to its position at setup

Your weight has gone  so much towards your trail heel so that the toe of your trail foot has come off of the ground

To fix this maintain more of the brace of your trail knee going back

 

The reason why you are OTT is that you do not understand the correct RIGHT ELBOW move in transition . It moves down and TOWARDS  the TARGET LINE  in early transition . To do this requires that right elbow must move faster than the hands in early transition

Video looks like a black silhouette swinging, so can't comment on your turn, but you have an extremely out to in path. watch your right shoulder on the downswing. It gets thrown out and away from you body making you come over the top. On the downswing, think about having the right shoulder work down instead of out.
