Driver went from Hook to Slice in <6 months, help!

First off, I have 2 videos but they were filmed sideways and I cannot figure out how to flip them, both on VLC and Youtube. Should I post them anyway?

 

I got the Adams Fast 12 LS in December. At first I had a hook, so I practiced swinging out on the downswing, and the hook turned into a nice draw. Lasted for 2 months, then it started straightening up. I thought that was odd, but I played it. 2 months later straight went to a slight fade--I thought that was very odd, but played it also. Now for the last two weeks it has been a dead slice. I even went to the range a few times and all except two swings were slices.

 

I can't figure out what might be happening? I don't think I changed anything in my swing? Though I will admit my back has been sore lately and while it doesn't seem to be affecting (feeling wise) my golf game, maybe it has slowed down my swing where my body can't keep up with my stiff shaft on the driver. Thanks for any help.

When I start slapping at the ball, my body is sliding laterally toward the target. So at impact, clubface is open. You could check to see if your are doing the same. Wheh that happens, I just focus on keeping my head in the same spot. If you have any type of injury, your body could also be putting you out of position or throwing off timing, but I would check to see if you are moving laterally towards the target.
