I'm far from an expert, so take this all with a grain of salt, but:

 

- Your grip looks relatively ok, so I don't necessarily think it's that. Somewhat strong, but don't think that's the reason

 

- What I do see is that the club face stays pretty closed as you take it away from the ball. If you look at club parallel on the backswing, the face is basically pointing to the ground. It should be closer to aligned with your spine. You may want to modify your take away a bit to get a better match up

Posture is an issue. Too bent over.    Looks like you bring hips toward ball in backswing and the that right leg kicks out early and the entire pelvis moves out to ball - so path goes out to the left and for you a lot - recipe for hooks.    Find a good instructor and take lessons starting with setup.

You are standing too far from the ball. Moving slightly closer will result in less bend towards the ball.
At the top your clubface is very closed , so it is no surprise that you are hitting hooks. Brooks Koepka has his clubface in a similar position at the top with his driver , but he hits the ball relatively straight. So how does he do it?

In transition he externally rotates his right shoulder , resulting in his right elbow moving slightly towards the TARGET LINE as it moves down . This action opens the clubface , shallows the shaft and most importantly moves his right elbow in front of his right hip. 
You fail to externally rotate your right shoulder causing your right elbow to move DOWN without it also moving out towards the target line . This results in your clubface  remaining closed and your right elbow being behind your right hip. That causes problems at impact .

So try to hit some freezers with your trail elbow glued to your trail seam line on your shirt.  Or feel like your trail elbow is connected to the trail seam line of your shirt at the top of the backswing.

 

So don’t go any farther than this on your backswing.

Like others have said you’re  bent over a lot which means the ball needs to be too far away from you. The over swing and closed clubface are also good points but start from set up and then see how it looks. You can’t do several things at once trust me:-)

 

set up differently hit 3/4 swings and take it from there.

Finally got out to practice today and took some videos working on setup and shortening my swing.  How does this look?

have a similar issue...usually b/c my grip gets too weak (yes too WEAK) and downswing is too far from the inside, which means you have to rotate forearms like crazy to try and square it...looks like your grip is a little on the weak side even though as others have mentioned you have it a little shut at the top.   Slicefixer has a good drill that helps get me out of the hooks from time to time. basically, take a 6 or 7 iron and hood it 45 deg closed.  then try and hit the ball straight.  will take some time, but will move your hands well forward and impact and open the face some...just gives you a good feel.  you could also try swinging a little more left with the mental image of never letting the toe catch the heel.  your swing looks pretty solid though.

Not the best dtl video but set up looks much better to me, a good guide from the coaches is having armpit knees and balls of feet in a line. Good work.

 

Get the camera in line with your hands and get the v1 app or others so you can check what you are doing more accurately. You are nowhere as bent over as you were. The pitch of the shaft in transition looks good too but I'm no pro.

 

From the face on the club face looks strong which isn't necessarily a bad thing unless it's staying that way and you're pulling it. Cant see the face in that video
