I'm seeing a combination of three core elements that are leading to your overall OTT delivery:

First you have this rocking back on to your heels in your takeaway which sets up a lot of potential problems, the most common of which is the simple reaction to this which is to move towards your toes at some point in the swing. Intentionally unbalancing yourself in any capacity is bad as you need to be balanced and stable to create the necessary forces in a "correct" golf swing.

Up into your backswing we see another more "extreme" kind of thing similar to the rocking back towards your heels in that you appear to be resisting any kind of wrist set/hinge. This is a problem because it gets the club working flatter/deeper behind you which in turn creates a reverse loop at the top of your swing that pushes you into an OTT direction (more on that below). Here is a good primer on how you want to feel your wrists setting correctly.

Now your hands start to change direction, moving straight up still without wrist set. We also see the beginning of you losing depth and coming towards the ball, likely a reaction to your heel shift in the beginning.

Up to the top we finally see wrist setting occur (very late) and more movement towards your toes now, re-balancing but in such a way that your momentum is now shifting concerningly towards the ball.

Down into impact we see a big move towards the ball with the right side as both your right knee and hip come straight out. The lower body is generally driving in the wrong direction here, out towards 1-2 o'clock if the target is 12 o'clock. Pros drive in the opposite direction, towards 10-11 o'clock.

Overall its a layered issue. The heel shift in the beginning unbalances you, the lack of wrist hinge gets your path a little wonky going back and the corrections that come after (hands coming straight up, late wrist set) seems to negatively reinforce the rebalancing that happens in transition that sets your momentum working towards the ball too much. The alignment stuff @tthomasgolfer605 mentioned above is relevant too based on your intended target vs. where you're actually aiming.

Proper wrist set via the video I linked earlier should be looked at first coupled with a general neutralizing of your alignment. Lower body stuff can be looked at next with an understanding that the interconnected nature of all this means you're going to hit some really bad shots for awhile. Start smaller/slower and take lots of video.

Everything is aimed right. Your Subconscious knows this and built in compensations. Now you have to train your aim and let your brain rewire itself. Takes a while, but it's worth it. I'd start there before getting into mechanics.
